If you have an emergency

If you have a lifethreatening emergency, please dial 911 or go immediately to the nearest hospital emergency room or labor and delivery. When you arrive ask them to contact your Women’s Health Now physician.

If your condition is less serious, but still need to talk to one of our team members, call our office at (480) 888-7271. If you reach us during office hours, the physician will call you personally or will deliver instructions through a team member. If you reach us after hours, you will be provided with instructions for contacting the “ on call” physician.

Insurance

We accept most insurances. Please contact Women’s Health Now to discuss the full-range of insurance selections, out-of -network benefits, and cash pay options.

Forms

There is a $20 charge for completion of disability and other forms such as Family Medical Leave Act (FLMA).

The Women’s Health Now Patient’s Responsibility

At Women’s Health Now we believe that the more you know about your own health and take an active role in researching your health issues, the better your care will be. We ask that patients take responsibility for:

Knowing their personal health history and keeping a written medical record that includes: • Past medical and surgical history • All current medications and any known medication allergies or intolerances. • Knowing their family history in particular any history of cancer or chronic illness. • Knowing date of last pap smear and mammogram if applicable.

Being on time for visits . Arriving late not only disrupts our schedule but also the schedule of each patient who follows you. Please be courteous and arrive on time.

Please notify us in advance of a cancellation if possible. Failing to appear for an appointment not only disrupts our schedule, but also disrupts the scheduling for other patients. Women’s Health Now charges $50 for any missed appointment or appointment not cancelled within six hours of the appointment start time for any reason.

Patient participation and feedback are essential to the quality of healthcare at Women’s Health Now. If you feel we have somehow not met your healthcare needs we would like hear from you in order that we may remedy the situation.
