I will send you an online intake form for you to complete prior to your first visit. This information helps me understand your child’s medical and feeding history which I will review prior to the evaluation.
THE EVALUATION
Next in the therapy process is an in-person evaluation at my office that typically lasts 1-2 hours. At this time we will discuss your goals and challenges and I will watch your child eat, drink, play, etc. At the end of the evaluation, we will agree on the goals for your child’s therapy and schedule your first therapy session.
For feeding evaluations, please bring the bottle(s) with nipples that your baby likes the most along with formula and/or expressed breast milk. If your child is old enough to eat baby or table foods, please bring your child’s two favorite and two least favorite foods and the utensils that you typically use. For initial speech evaluations, you do not need to bring anything.
FEEDING AND/OR SPEECH THERAPY
Therapy sessions are typically scheduled every 1-2 weeks for 45-60 minutes. The actual frequency of visits and overall duration of the therapy plan varies from child to child depending on needs and things you are able to do at home.
To get the most out of the therapy process it is important that trust is established between the child, parent and therapist. I will work to build trust with you and your child. Trust is the foundation to children being willing to engage in any activity, especially eating and drinking. It can take a few sessions to build that trust and I may ask you to help with certain steps during that process.
What is the role of a parent during the therapy process?
Therapy for children is a team effort between the parent and the therapist. At each session I will review the steps you should be taking at home between our sessions. Being engaged during the therapy sessions and committing to the home program is critical to helping your child progress as quickly as possible. Your home program will include achievable goals that take into account family needs and routines. In other words, I will not recommend that you complete a certain activity at a time of day that does not work for your family.
Where do the therapy sessions occur?

Sessions take place at my office which is located at 200 W. Main St, Suite 102, Mount Horeb, WI 53572.
Do I need to attend the therapy sessions with my child?
Yes. It is critical that the parent or caregiver who will be carrying out the at home portions of the care plan be present in the therapy sessions to achieve the best results.
Do I need to bring anything to my initial feeding evaluation?
For your initial feeding evaluation, please bring the bottle(s) with nipples that your baby likes the most along with formula and/or expressed breast milk. If your child is old enough to eat baby or table foods, please bring 3-4 foods that your child prefers. In addition to food, please bring what your child prefers to drink from and their favorite beverage.
For initial speech evaluations, you do not need to bring anything.
How often will I need to bring my child back for visits?
Sessions typically occur every 1-2 weeks. The frequency of therapy may change throughout the course of treatment.
How long will my child need therapy?
The length of time that your child will need therapy depends on your child’s needs and your goals.
How will we know when therapy is complete?
Therapy will continue until your goals are reached and you feel confident in your ability to help your child at home.

Do you accept insurance?
I do not accept insurance but I will provide you with a detailed invoice to submit to your carrier if desired. If your insurance plan does offer full or partial reimbursement for out-of-network services, a physician referral is sometimes required. I recommend that you reach out to your insurer directly to confirm the appropriate steps and expected reimbursement for your particular benefit plan.
How much does feeding and/or speech therapy cost?
Evaluations are $300 and last 1.5-2 hours.
Treatment sessions are $125 and last 45-60 minutes.
I will provide you with a Good Faith Estimate of the cost of therapy for your child after we discuss your child’s needs.
What is a Good Faith Estimate?
Under the No Surprises Act that went into effect January 1, 2022, health care providers are required to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
What payment methods do you accept?
I accept credit, debit, FSA or HSA card – you can enter this information securely online during the intake process so you don’t have to worry about remembering to bring your card to each visit.
What is your cancellation policy?
No shows or cancellations within 24 hours of your scheduled visit will be charged at the full hourly rate unless your child is sick or there are other extenuating circumstances.
